Phlebotomy Recertification in Washington

The State of Washington requires certification to practice as a phlebotomist.

Continuing Education Requirements:

• To renew your license, visit the Washington State Department of Health license renewal page by clicking HERE

NHCO Renewal:

• To maintain your certification, you must complete the continuing education or employment time and fill out the NHCO recertification application, which can be found here.

• You must complete 12 hours of continuing education every 2 years. However, if you have 1,040 hours of verified work experience in the prior two years, you can renew without continuing education. This is part-time work, consisting of 520 hours per year.

• Renewal Fees:

  • $100 per renewal period of 2 years

  • $25 reinstatement fee if the certificate is expired
